Plus you can attach pictures, scanned doctor notes, etc.<BR/><BR/>Some programs have recommended immunizations, growth charts, etc.<BR/><BR/>Take a look at http://www.HealthFrame.com or http://www.CapMed.com or even more from http://www.myphr.com.<BR/><BR/>The paper form is cute, but it may not be as useful in the long run...Anonymousnoreply@blogger.com
